This updated edition brings additional experience with which to assist the reader in delivering optimum safety and efficiency of performance of the HAZOP team. HAZOP is the most widely-used technique in the process industries for the identification of hazards and the planning of safety measures.

Technologists and the public look at risks in different ways: technologists try to find a way of estimating the size of each risk and then try to deal with the bigger ones first; their decisions may not be right but the reasons for them are usually made clear. Hazop and hazan

The development and use of the fundamental risk assessment process, the HAZOP Hazards and Operability Study , was originated in the 's by Trevor Kletz at ICI Industries in Great Britain, and has been a developing process both in its content and the extensiveness of its application during the last 50 years. In particular, the HAZOP process has evolved to be applied at different stages of design and execution such that industry would specifically apply the process to its older Brownfield terminology facilities where they can show significant reduction in chemical related incidents with use of the HAZOP process in their mature plants. This demonstrates the existence of a latent risk in these facilities which can occur for several reasons but which is reduced by a HAZOP process. The author's use career experiences and recommended practices to advocate the HAZOP in the operations phase which is also known as the OESR, the Operational, Environmental and Safety Review while illustrating through case histories the use of this tool. Conclusions and recommendations are offered for further application and risk reduction in operational facilities. Knowledge management includes a library of understanding of the modes of failure such as corrosion, erosion, loss of material properties, cracking, fatigue, other failure modes and related aspects which are critical in deepwater projects. This is due to repair, retrofit, or re-habilitation being costly or impossible.

Hazard and operability study

During the s to s, ICI personnel were responsible for major innovations in chemical process safety, which ultimately affected the safety culture in all sections of the chemical industry. This change improved the quality and robustness of operating processes, not only resulting in improved profits, but also saving lives. These metals are so violent when they split Our very walls can scarce stand up to it. But six centuries later, the attitude to safety and accident investigation had changed very little, the emphasis still being on blame!

A hazard and operability study HAZOP is a structured and systematic examination of a complex planned or existing process or operation in order to identify and evaluate problems that may represent risks to personnel or equipment. The intention of performing a HAZOP is to review the design to pick up design and engineering issues that may otherwise not have been found. The technique is based on breaking the overall complex design of the process into a number of simpler sections called 'nodes' which are then individually reviewed. It is carried out by a suitably experienced multi-disciplinary team HAZOP during a series of meetings. The HAZOP technique is qualitative, and aims to stimulate the imagination of participants to identify potential hazards and operability problems.
